warrant that I possess all the rights, powers and privileges of a parent or legal guardian necessary to execute this document with binding legal effect.
I, the parent or legal guardian of the child named above, do hereby grant my permission and consent for the said child to attend and participate in events and activities of Gospel Community Church, both on and off church grounds, including the necessary transportation to and from these events and activities. I certify and affirm that I have been completely and thoroughly informed that by attending Gospel Community Church (GCC), my child will be allowed to participate in certain activities which carry with them a degree of risk and danger.
Examples of risky and dangerous activities include, but are not limited to: physical activities, both indoors and outdoors; sports, both informal and organized; use of recreational equipment; field trips, both on and off GCC property; travel by automobile, airplane, van or bus; activities in or around water, including swimming and boating; hiking, camping; etc. I acknowledge and understand that Gospel Community Church may offer other activities not listed that present similar risks or dangers to my child, and also consent to those activities, whether the activities engaged in are free or if a fee is charged.
I further assume, on my child’s behalf, all risk in connections with said activities for any harm, injury, or damages that may befall my child as a result of my child’s participation in the activities, whether foreseen or unforeseen. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Gospel Community Church and any of its paid staff or volunteers responsible for any accident that may occur on the way to, from, or during any event or activity, and from any and all claims, demands, or causes of action, which are in any way connected with my child’s participation in these activities or use of Gospel Community Church’s equipment and facilities.
I further consent to the examination or treatment of my child by a duly licensed physician or health care professional to provide medicine, health care services, and medical care deemed necessary by them and Gospel Community Church’s agents, volunteers and employees as a result of any participation in these activities. I also give consent for my child to receive medical care if the medical treatment or procedures are immediately or imminently necessary, and any delay occasioned by an attempt to obtain my parental consent would reasonably jeopardize the life, health, or well-being of the child affected, or I cannot be personally contacted. I agree to pay for any and all medical expenses incurred as a result of the use of this consent.
Should it ever be necessary for my child to be returned home due to disciplinary action (as deemed necessary and appropriate by GCC leadership), I will also assume all costs of transportation and be responsible to pick them up if they were driven to the event.
I understand this form is effective and binding for the entire duration my child is involved in GCC Student Ministries, and/or chooses to participate in any event connected to Gospel Community Church.
By signing this document, I acknowledge that if my child or anyone is hurt or property damaged during my child’s participation in these activities, I waive any and all rights to maintain any lawsuit against Gospel Community Church, its agents, volunteers and employees on the basis of any claim from which I have released them herein.
I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect. I have fully informed myself of the contents of this LIABILITY RELEASE by reading it before I signed it.
